Preferred Physical Therapy LLC provides SMS messages to patients regarding appointment scheduling, appointment reminders, follow-ups for Discovery Sessions, and responses to patient inquiries.

2. Opt-Out Instructions

You can cancel the SMS service at any time. Just text STOP to unsubscribe. After you send the SMS message STOP to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.

3. Message & Data Rate Disclosure

Message and data rates may apply for any messages sent to you from us and to us from you. Message frequency varies based on your interaction and schedule.

4. Carrier Liability Disclaimer

Carriers are not liable for delayed or undelivered messages.

5. Age Restriction (18+)

By providing your phone number, you represent that you are at least 18 years of age and have the legal capacity to agree to these terms.
